Request a copy of your policy or declarations page

Opening an online account with us is the easiest and quickest way to get a copy of your policy. Otherwise, you can order a copy of your policy and we will mail it to you. You can also request a copy by contacting your agent or calling us at 800-543-2644.

Make a change to your coverage

You will need to contact your agent for any change that affects your coverage, limits, or the deductible. A contact number for your agent is printed on your policy's declarations page.

Cancel your policy

It's best to discuss a cancellation with your agent, but you can also deal directly with American Modern. Your cancellation request must be made in writing including your signature. Return it to us by mail or fax.

We cannot accept cancellation requests requested by email.
